Uninstall survey

As the extension maintainer
I want to know why users are uninstalling the extension
So that I can improve the extension to meet their expectations

Given I have toggle-youtube-comments installed
When I uninstall the extension
Then I am taken to an online survey

Note there's also an event for when the extension is being uninstalled.

Can use Google Forms, SurveyMonkey, TypeForm, etc.
