0.15 & 0.16 issues

Issue description
#1 hotbar does not save the selected items in 0.16
Example: if i select a sword and put it in my hotbar in survival then open a chest then close it my hotbar resets to before i put the sword there.
#2 while playing in survival sometimes items start dropping out of hotbar
OS and versions

• Genisys: 1.9.3
• PHP: 7.0.2
• Server OS: Ubuntu 16.04.1 LTS
• Game version: 0.16.1

@ishitatsuyuki Just checked both of those links and my issue does not mention anything about what's inside those links, ive also checked them ahead of time.

@ishitatsuyuki I see that i did not do the right issue report format, and have fixed it. Sorry about that. I'm guessing this issue will reopen or it will not because of my mistake?

@ishitatsuyuki One of the issues was a dupe, welp my mistake. removed it i only saw the enchant issue already reported in new issues and closed issues. sorry
