Re-write core algorithms in C++

Re-coding the core algorithms for the estimators (i.e., log OR, RMST, etc.) in C++ would be a valuable effort. These are the slowest parts of the package because they are iterative (especially for survival). 

Likely not to occur in the immediate future.
