Add user story issue template

Description

@jasongrout identified that practitioners shouldn't need to sign up for an interview to submit users stories.
This new issue template helps users submit their stories on their own time so we can evaluate our implementations against the metrics (context switches, time spent, number of steps) in their story.

Checklist

• PR has a descriptive title and content.
• PR description contains references to any issues the PR resolves, e.g. Resolves #XXX.
• PR has one of the labels: documentation, bug, enhancement, feature, maintenance
• Checks are passing.
  Failing lint checks can be resolved with:
  • pre-commit run --all-files
  • jlpm run lint
