Combined fixes from issues #3142, #3610, #3726

  • formatting and paragraphing changes
  • correct subchapter numbering in pli-tv-pvr root text
  • update translation in segments with no root text

Are changes to html/, reference/, and root/ still manually put into published? If so, I'm happy to collect all of those changes from these few pull requests and open another PR into published in case that would be helpful.
